Creating Of Custom Balance Sheet

How do I create a balance sheet that looks like this:

Balance Sheet – 4 columns (let’s say at Jun 30 2010 I would report)

-current month (Jun 30 2010)

-previous year, same month (June 30 2009)

-previous year, year-end balance (Dec 31 2009)
